LIMITED PRODUCT WARRANTY
KlearFX LLC ensures that its aligners and retainers are free from defects in materials and craftsmanship when used correctly for their intended purpose. Upon receipt and within the initial 15 days, it is important to inspect the product. If deviations from the virtual treatment plan or manufacturing processes are identified, the laboratory will repair or remake the case without compromising the product warranty.
Beyond the initial 15-day period, KlearFX LLC will, at its discretion, repair or replace any device found to be defective in materials or craftsmanship despite adhering to the original prescription’s instructions. This limited warranty exclusively covers material and craftsmanship defects in the original devices and does not extend to breakage or loss during regular customer usage. Instances of damage resulting from accidents, misuse, improper assembly, inadequate tooth or tissue support, incorrect adjustments, abrasion, bruxism, or inadequate dental hygiene are not covered.
TREATMENT WARRANTY
Treatment warranties become effective upon the dispatch of the initial aligners to the customer. All treatment plans, (except the “Comprehensive Treatment” which is cover for one year), carry a six-month warranty. These warranties encompass mid-treatment corrections or final refinements. However, any warranty becomes void if the patient deviates from the prescribed treatment plan. To avail themselves of treatment warranty benefits, patients must adhere to the aligner usage instructions provided by the doctor and included in the packaging. Furthermore, doctors must oversee patient compliance and confirm that the case is progressing as expected. The suggested monitoring method, if employed by the doctor, may serve as evidence of compliance. Alternatively, if the doctor opts not to use the suggested monitoring method, they must submit documentation and photos of the patient using the aligners to demonstrate proper supervision and adherence to the treatment plan.
Patients who fail to adhere to the treatment plan or abandon treatment altogether forfeit any warranty benefits. Additionally, warranties do not apply if interproximal reductions (IPR) were not performed or if restorative treatment commences after the simulation treatment plan has commenced. It’s crucial to note that treatments are tailored to individual patients, and any unused items from a treatment plan are non-transferable. Save for the provisions outlined in this section, KlearFX LLC Dental Lab LLC does not explicitly, implicitly, statutorily, or otherwise warrant the aligner treatment options, products, or services, nor the outcomes associated with them.
PRODUCT CANCELLATION FEE
A cancellation fee of ($50.00) will be levied if an order for a digital treatment plan, retainers, replacement aligners, or refinement is canceled within two working days of confirmation.
